1. Patient Identification: Foundation of Safety

Critical Practice Using two patient identifiers (name and medical record number, or name and date of birth) before ANY medication administration is mandatory to prevent wrong-patient errors.

Correct patient identification is the critical first step in safe medication administration. The standard of practice requires using two independent identifiers to confirm patient identity before administering IV medications. The identifiers should be verified against the medication order and the patient's wristband or identification. The Joint Commission identifies wrong-patient errors as a sentinel event with serious consequences. Never rely on room number alone as an identifier, as patients may be moved. Never trust verbal confirmation by family members—always verify independently. The two-identifier approach has been shown to prevent the vast majority of wrong-patient errors. This fundamental safety practice must be performed consistently and without exception, regardless of how busy the environment or how well the nurse knows the patient. Systematic adherence to proper identification protocols is a hallmark of safety culture.
